1) “Caregiver” release forms (found in the 2nd tab on this current page). You may also upload your picture here if you did not on the registration form. This is for OHM staff use only. This is used to allow the ministry and welcome team to recognize you once on campus (to put a face to the name). 2) Payment plans (if you haven’t paid in full yet…i.e. if you paid only a deposit, you will have an outstanding balance). 3) Homework – how to be prepared for The Journey 4) Schedules, check-in information, and… 5) Lodging/Meals – for additional lodging and meals outside of the event timeframe. Bookmark this page in your web browser so that you have access to the material here whenever you need. This is your “Homebase” for everything regarding The Journey. If you are currently under the care of a counselor or therapist, you will need to have them sign a release form. Once your form is signed, upload it on this page, email to darin@ohmin.org or text a photo to +1 (269) 389-9491. Typically PDF would be the preferred format, but you may use jpg, gif, png, etc.
